A passport is required for this trip. Your passport must be valid for at least six months after the scheduled return date of your vacation. Bring with you a photocopy of the pages of your passport that contain your personal information and extra passport photographs. These are useful in the event of loss.
Please note that you are required to have travel/health insurance to travel to the Czech Republic. The Czech Republic requires that all travellers entering the Czech Republic provide documentation of a minimum of $38,000 in medical coverage in the form of health insurance or travel protection. The minimum coverage of $38,000 must cover possible medical expenses and hospitalization charges as a result of accidents or unexpected illness while travelling abroad, including repatriation to your country of legal residence or the country that issued your passport.
While travelling in the Czech Republic you may be asked to show proof of the minimal medical coverage of $38,000. A medical insurance card from a private insurer is sufficient proof. Medicare cards, however, are not acceptable as Medicare does not cover medical expenses abroad.
